Sports Physical, PIAA Form completion

Last Names L-Z 6pm

WHERE:

North Penn High School Auxiliary Gym

WHY:

All athletes anticipating on being part of the 2024/25 season are required to have section 7 of the PIAA form completed on or after May 1, 2024. 

WHAT TO BRING:

a Parent must accompany the student with a printed PIAA Form, Section 5 must be complete. 

This form is necessary for all school sports registration. The form must be uploaded toBig Teams, paper copies will not be accepted.
